Starting September 1, 2025, a new procedure for the provision of medical care using telemedicine technologies will come into force in Russia. Ministry of Health Order No. 193n of April 11, 2025 introduces a legal framework for the widespread use of artificial intelligence (AI) systems in diagnosing and consulting patients.
The main change is the recognition of the legitimacy of the use of AI-based medical decision support systems (MDSS) for delayed telemedicine consultations. These systems, registered as medical devices, will be able to analyze a patient's medical data without personal contact between doctor and patient, reducing the time for diagnosis and treatment selection.
The rules for remote health monitoring have undergone significant changes. Now not only treating physicians, but also other specialists involved in monitoring can react to critical changes in a patient's health indicators. An automatically triggered system will notify the nearest doctor on duty about alarming changes, which will speed up the provision of assistance.
An important point is the introduction of mandatory certification of telemedicine consultations with the enhanced qualified electronic signature (UKEP) of a medical worker. All materials generated during online interactions will be archived electronically for at least one year, and audio and video recordings will be stored for five years.
The order eases the paperwork but strengthens technical and legal requirements. Using AI systems without proper registration could threaten clinics with license revocation. There are still risks of personal data leakage when using external platforms that are not integrated with the unified state healthcare information system.
The introduction of new rules promises to increase the availability of quality medical care for residents of hard-to-reach regions, but requires improvement in the technical equipment of medical institutions. According to the research company Monitor, only about 40% of outpatient clinics and polyclinics are able to correctly transmit ECG data in remote mode.
